Parent Permission Forms

​​Parents must complete three consent forms every year for each of their children.  

These forms include:

  • Student Acceptable Use of Technology Agreement
  • Collection, Use and Sharing of Personal Information
  • Impromptu Walking Field Trip Informed Consent
